David Beguin 92cb648882 [FIX] survey : externalise survey web assets to avoid multiple load at website install

Since f5625969be, web assets in survey module
was loaded twice when website (and website_survey) is installed.

Thoses web assets are now externalised in another template that is loaded
only if survey is installed without website installed.
