Subject: [PATCH] [FIX] website_sale: fix the missing action on checkout form

Remove wrong attr name '/shop/checkout' and add the action '/shop/address'.

It allow others routes to render the form, and that the submit form is handled
by the default checkout process.

Todo: make this 'action url' as an argument
